First, identify which kind of examination you need
- A university, scholarship or administrative requirement: ask that organization which form, examination provider and supporting documents it accepts before paying for tests.
- A personal preventive health check: compare our packages and listed examination details. A standard bilingual checkup report is not automatically a substitute for an administrative form.
- Both: get the required administrative list first, then ask whether any results can be shared between the two purposes. Do not assume acceptance or buy duplicate tests in advance.
A practical preparation checklist
- Keep the instructions supplied by the requesting organization together with the blank form.
- Confirm the accepted provider, submission deadline and any requirement to retain originals.
- Ask the clinic which identification, photographs and supporting reports to bring.
- Complete personal details accurately; leave clinical findings and certification to the examining clinician.
- Before leaving, ask the clinic to check the required signatures, dates, stamps and attachments against your instructions.

How long is the form valid?
Check the rule for your own application. For example, Shanghai's 2026 guidance for the Chinese Government Scholarship Bilateral Program states that applicants planning to study for more than six months submit the examination copy, retain the original, and use results valid for six months. It also specifies completion and certification requirements. That program's instructions should not be treated as a universal rule for every visa or visit. Read the official program guidance.
Can I use a regular health checkup package?
Only if the requesting authority accepts the provider, completed form and examinations. Confirm this separately. Our published package prices describe preventive checkups; they are not quotations for government certification or immigration processing.
Do I need to send you my completed form?
No. The download is available without contacting us. If you also want a personal checkup, start with your city, dates and budget. Do not send passport copies, completed examination forms or medical results through the general enquiry form.
